A wound care partner for your facility

We work with skilled nursing facilities, assisted living communities and board-and-care homes across Los Angeles County. Residents get specialist wound care without leaving the building.

What we offer

How we support your facility

In-facility wound rounds

Our clinicians see residents on a set schedule, usually weekly, and treat wounds as the physician ordered.

Staff training

Short, practical sessions for nurses and aides on wound checks, dressing changes, turning and pressure injury prevention.

Clear documentation

Measurements, photos and notes at every visit, written to fit into your charting and care planning.

Coordination with your nurses

We meet with your nursing team during rounds, share the care plan and make sure orders are clear for every shift.

Documentation

Documentation that supports survey readiness

Surveyors look closely at how facilities prevent and treat pressure injuries. Under CMS F-tag 686 (F686), residents should get the care they need to prevent pressure injuries and to help existing ones heal.

Our wound notes include measurements, photos, wound descriptions and treatment changes at every visit. This supports your team’s documentation of pressure injury prevention and care. Your facility’s own policies, assessments and care plans remain your team’s responsibility.

Prevention

Prevention programs

Stopping a wound before it starts is better for residents and for your team.

  • Help spotting residents at higher risk for pressure injuries
  • Practical tips on turning schedules, cushions and support surfaces
  • Skin check routines staff can use every day
  • Nutrition and hydration reminders that support healthy skin
  • Foot checks for residents with diabetes

Working together

How a facility partnership works

  • We meet with your director of nursing or administrator to understand your needs.
  • We agree on a visit schedule and how referrals will be sent.
  • We verify each resident’s coverage before the first visit.
  • We send progress notes to the attending physician and share updates with your nurses.
  • We check in regularly to see what is working and what could be better.

Questions

Frequently asked questions

What types of facilities do you work with?

Skilled nursing facilities, assisted living communities and board-and-care homes in Los Angeles County.

Who orders the wound treatment?

The resident’s physician. We treat under physician orders and call the physician before changing the plan.

Can you train our staff?

Yes. We offer practical training on wound checks, dressing changes and pressure injury prevention. Ask us about scheduling a session for your team.

Does working with you make our facility compliant?

No outside partner can do that for you. Our notes are written to support your team’s documentation, but compliance stays with your facility’s own policies and care.

Ready to get a wound looked at?

Tell us about the wound and we will call you to schedule a visit at home or at the facility. Physicians and facilities can send a referral online or by fax.
