• For referrals: the referring contact’s details and basic patient details.

Please do not send detailed medical records through this website. If we need them, we will request them through a secure channel.

How we use it

• To contact you, verify insurance and schedule visits.
• To coordinate your care with the referring physician or facility.

We do not sell your information or share it for advertising.

How it is sent

Forms are sent encrypted (HTTPS) to our intake team by email. This site does not use tracking cookies or advertising tools. If you turn on “Larger text”, your browser remembers that choice on your own device.
