Why refer

Why providers refer to WoundCarerX

We work with physicians, podiatrists, nurse practitioners, physician assistants, home health agencies and hospital discharge planners.

Care where the patient lives

Patients who can’t easily travel still get regular wound care, so visits don’t get missed.

You stay the physician of record

We treat under your orders and call you before changing the plan. Your patient stays your patient.

Clear, regular updates

You get progress notes with measurements and photos, so you can follow healing without extra work.

Quick follow-up

We contact the patient or facility within 24–48 hours of a complete referral to verify coverage and schedule.

Support for home health and discharge

We coordinate with home health nurses and facility staff, which helps when patients leave the hospital with an open wound.

Send a referral

What to include in a referral

Communication

What you get back

Progress notes

Notes after visits, sent weekly or as you prefer, by fax or another secure method.

Measurements and photos

Length, width and depth at every visit, with photos, so you can see the trend over time.

A call when things change

We call you when the plan may need to change, when we see signs of infection or when the wound is not improving.

Your patient, returned to you

When the wound heals, we send a discharge summary with a prevention plan and return the patient to your care.
